Innovative Australian Care (IAC) believes that every person deserves care that recognises their unique story, culture, and complexity. Today, we’re proud to share the journey of Karina, a resilient Aboriginal woman who, with the right support, has found stability, independence, and renewed hope.
Meet Karina
Karina is an Aboriginal woman living with nine diagnosed disabilities, including a head injury, mild dementia, panic disorder, diabetes, and a history of suicide attempts. Her health challenges are extensive, and like many individuals with complex care needs, she was caught in a cycle of hospital readmissions and inconsistent support.
What Karina needed was more than just services, she needed a team who would listen, advocate, and provide flexible, culturally aware care that adapted to her changing needs.
A Comprehensive, Compassionate Approach
That’s where Aya and the team at IAC stepped in.
Working collaboratively with a Positive Behaviour Practitioner, IAC developed a trauma-informed, in-home support plan tailored specifically for Karina. This approach focused on consistency, compassion, and cultural sensitivity, helping Karina regain control of her daily life and health journey.
Key Elements of Support
Here’s how IAC’s innovative and person-centred care model made a lasting impact on Karina’s life:
Comprehensive In-Home Support
We provided multidisciplinary care that addressed both clinical needs and mental health, all delivered within the comfort and safety of Karina’s home.
Crisis Prevention & Early Intervention
Our team was trained to respond proactively to emerging issues, helping to de-escalate potential crises before they required hospital intervention.
Flexible Rostering & Support Hours
Karina’s schedule was tailored around her needs, ensuring she received high-quality care without exceeding her NDIS budget.
Collaboration with Allied Health & NDIS
We worked hand-in-hand with specialist Support Coordinators, Allied Health professionals, and the NDIS to review her plan and advocate for appropriate, ongoing funding.
Innovative Use of NDIS Funds
Through strategic planning, we were able to maximise the effectiveness of her existing plan, allowing Karina to receive comprehensive support without compromise.
Positive Behaviour Support
With guidance from a Positive Behaviour Practitioner, we introduced personalised behavioural strategies that helped improve Karina’s emotional regulation, independence, and daily interactions.
The Impact: A Life Transformed
Thanks to this structured yet flexible approach, Karina has experienced profound improvements in both her physical and emotional wellbeing. Some of the key outcomes include:
- Stabilised clinical and mental health
- A significant reduction in hospital readmissions
- Greater satisfaction and trust in her care team
- Improved management of her multiple diagnoses
- A renewed sense of dignity, safety, and self-worth

Karina’s story is a powerful reminder that with the right support, people with complex needs can not only survive, they can thrive.
